What they do
It names the clash between the high odds that alien life exists and the fact that we've detected exactly none of it.

Over lunch with colleagues Teller, York, and Konopinski, sparked by a New Yorker cartoon about flying saucers stealing trash cans, Fermi asked, 'Where is everybody?'
Why it matters
The engine uses it not to argue about whether aliens exist, but as a clean test case for measuring pressure at the largest possible scale.
